ACCIDENTAL DEATH & DISMEMBERMENT

Included in Cigna Basic Life.  Provided at no cost to employee

SHORT- TERM DISABILITY PLAN

All active, full-time employees obtain 60% of their basic weekly earning, not exceeding $2308. Provided at no cost to employee.

LONG -TERM DISABILITY

All active, full-time employees obtain 60% of their base monthly earnings, not exceeding $10,000 per month. Provided at no cost to employee.
